5 Essentials survey

The 5Essentials Survey will be administered online at https://survey.5-essentials.org/illinois/ through April 2, 2021 for parents, students, and teachers.  

Parents:  To receive parent data, a school must achieve a response rate of 20 percent from parents. If parents have more than one (1) child and they attend the same school, they may elect to participate multiple times. Parents with children enrolled in multiple schools may participate in the survey for each school. Only one parent per household may participate in the survey. 

• To complete the parent survey:
  • Visit https://survey.5-essentials.org/illinois and select Parent Survey.
  • Select the necessary language to complete the survey. (English or Spanish).
  • Enter requested demographic information.
  • Advance to survey questions.
  • Answer all applicable questions.
  • Click Submit Survey.

Students:  Student participants will be required to log-in using their state student ID and birth date to complete the survey. Note: The state student ID is a nine-digit number pulled by ISBE from the Student Information System (SIS).  

• To complete the student survey:
  • Visit https://survey.5-essentials.org/illinois and select Student Survey.
  • Enter state student ID and birthdate.
  • Select the necessary language to complete the survey (English or Spanish).
  • Answer all applicable questions.
  • Click Submit Survey.

Teachers:  Teachers will receive an invitation email with unique username and password to log-in to the survey and participate. A teacher survey report will be generated if at least 50 percent of teachers respond to the survey.

• To complete the teacher survey:
  • Visit https://survey.5-essentials.org/illinois (this URL will be included in their invitation)
  • Enters their unique username and passcode, which has also been provided via email.
  • Answer all applicable questions.
  • Click Submit Survey

• Provisional teacher survey because a teacher is not listed on your roster:

  • Go to https://survey.5-essentials.org/illinois/survey/teacher/provisional.
  • Fill out required fields and advance to survey questions.
  • Answer all applicable questions.
  • Click Submit Survey

• Individuals in the following positions, who also teach, are also encouraged to participate:

  • Self-contained and subject-specific classroom teachers.
  • Instructional coaches and subject matter specialists.
  • Teacher aides and paraprofessionals.
  • Special education teachers working in a single classroom or across classrooms.
  • Counselors, librarians, speech language pathologists and other staff members who teach students
